Declaration Form

Prior to arrival, each traveler or head of family must provide information concerning goods/gifts that have been acquired abroad. Complete items 1 through 15 and include your signature and the date written as Day/Month/Year at the bottom of this form. U.S. residents are normally entitled to a duty-free exemption of $800 on those items accompanying them into the United States, while non-U.S. residents are normally entitled to an exemption of $100.

Arrival/Departure Card

This form must be completed by all persons except U.S. citizens, returning resident aliens, aliens with immigrant visas, and Canadian citizens visiting or in transit. Type or print legibly with a pen in ALL CAPITAL LETTERS. Use English. Do not write on the back of this form. This form is in two parts. Please complete both the Arrival Record (Items 1–13) and the Departure Record (Items 14–17). When complete, present this form to the U.S. Customs and Border Protection Officer.

Arrival/Departure Card: I-94w Form

This form must be completed by every non-immigrant visitor not in possession of a visitor's visa, who is a national of one of the countries enumerated in 8 CFR 217. Type or print legibly with pen in ALL CAPITAL LETTERS. Use English. This form is in two parts. Please complete both the Arrival Record (Items 1–11) and the Departure Record (Items 14–17). The reverse side of this form must be signed and dated. Children under the age of 14 must have their form signed by a parent/guardian.


VISA WAIVER PROGRAM INTRODUCES THE ELECTRONIC SYSTEM FOR TRAVEL AUTHORIZATION

Beginning August 1, 2008, the Electronic System for Travel Authorization (ESTA) will be accessible via Internet for citizens and eligible nationals of Visa Waiver Program (VWP) countries to apply for advance authorization to travel to the United States under VWP. ESTA will initially be available in English only. Other languages will follow. Effective January 12, 2009, all VWP travelers will be required to obtain an electronic travel authorization prior to boarding a carrier to travel by air or sea to the U.S. under the VWP.
